In gay couples where one partner is HIV-positive and the other HIV-negative, an undetectable viral load is associated with unprotected anal sex, according to a study conducted in Sydney and presented as a poster to the Fifteenth International AIDS Conference in Bangkok last week.

This finding adds further weight to a recently published meta-analysis of studies looking at the association between HIV treatment and sexual risk taking. This meta-analysis, which was reported on aidsmap. A serodiscordant couple is one in which one partner has HIV and the other has not.

Many people dislike this word as it implies disagreement or conflict. Alternative terms include mixed status, magnetic or serodifferent. When the statistical data from all studies which relate to a particular research question and conform to a pre-determined selection criteria are pooled and analysed together.

Insertive anal intercourse refers to the act of penetration during anal intercourse. Receptive anal intercourse refers to the act of being penetrated during anal intercourse. In the context of drugs or alcohol, withdrawal is when a person cuts out, or cuts back, on using the substance, also known as detoxification or detox.

In a context of sexual risk reduction, it refers to the insertive partner in penetrative sex withdrawing before ejaculation. It is not a particularly effective way to lower the risk of HIV transmission or pregnancy. Data were collected from gay men involved in two ongoing cohort studies in Sydney.

However, the investigators found that even when unprotected anal sex was occurring, men in serodiscordant relationships were making efforts to minimise the risk of HIV transmission.

However, when unprotected anal sex does occur in serodiscordant couples it is significantly associated with an undetectable viral load in the HIV-positive partner, younger age, and optimism about HIV. Even when unprotected sex does occur, men often make efforts to minimise the risk of HIV transmission, for example by withdrawal before ejaculation if the HIV-positive partner is the insertive partner.
